Shift: 8:00 AM - 4:30 PM Experience: • 24 months of Rehabilitation Unit experience is required. • 3 months of experience with Epic is required. Requirements: • Candidates must have a license prior to placement start. • This role will require floating to multiple units • Candidates must live at least 80 miles away from this facility in order to be considered a traveler when applying for this role. • COVID vaccination required for onboarding. Additional Details: Guaranteed Hours: 40 Monday through Friday Will work between the clinic OP and SNF. Closed during holidays, so alternate schedules will be available. Min 1 year outpatient experience Willing to float between outpatient clinics and SNF as needed Comfortable working alone in a clinic setting Evaluation and treatment of patients for physical therapy in outpatient and SNF setting as needed. Equipment utilized in the unit/dept. EPIC navigators notes, Casamba at nursing home. Required skills and minimum years of experience. Experience with outpatient and subacute helpful Shift or schedule requirements.
