Job Description:

A social services agency in New York City is looking to add a Spanish-speaking professional to join their growing Comprehensive Program as a Case Manager. Working under the Program Supervisor, the Case Manager will be responsible for both clerical and practical support.Responsibilities:The Case Manager will:Enroll 50 participants into the program and lead and manage 50 cases independentlyConduct timely intake, orientation, and assessment of all participants in order to develop service plans to help their families gain confidence, self-sufficiency, and greater connection to their communityProvide advocacy, outreach, and referrals to entitlement agencies, social service providers, and other programs on behalf of program participantsPerform related crisis intervention activities and escort participants to appointments as neededDevelop ISP plan with short- and long-term goals for participants enrolled in the programDevelop self-advocacy goals with participants enrolled in the programPerform follow-ups on bi-weekly basis.Qualifications:Bachelor's Degree in Human Services or relevant field required or completion of the Family Development Credential (FDC) Program within the first 6 months of employmentBilingual (English and Spanish)Great interpersonal skillsExcellent communication skills (written and verbal)Desired Skills:1+ year of Case Management experience in a Community-based settingCounseling credentials
