We provide our patients with Medical Health, Behavioral Health, Psychiatric Care, and Social Services.As a Vivia Health Social Work Consultant and Lead, you will have a direct impact on the development of the Behavioral Health department and services provided to our patients. You will manage a team of highly motivated and passionate Therapists. You collaborate cross-functionally with an interdisciplinary team to ensure we are providing the highest quality of care to our patients.What will your role entail?Manage a team of highly motivated, provide them with guidance, on-going actionable and measurable feedback, resolve issues and concerns, and assist in growing the teamAnalytics and reporting of population demographics and trends for each clinic and make recommendations to managementEnsure Vivia Health is meeting census goalsActively participate in weekly management meetingsReview therapists' charts including but not limited to ensuring accurate diagnosis, social history, care plan, and that there are measurable goals that reflect the patients care planThis role is remote within the Chicagoland area with monthly travel to each of the 3 clinic locations. This is a part-time role, and the hours will be approximately 20 hours a week with a flexible work schedule.What do you bring as a therapist?At least 5 years of experience managing and training therapistsLCSW or LCPC license required; open to applicants that recently received their LCSW license with 10 years of experienceBackground and experience working with Medicaid patients and directly working with a diverse demographic of patientsAttention to detailAbility to set clear expectations and goalsClear, direct, and concise communicationCollaborationExperience training therapists
