Deliver direct Safety and Permanency Services , as determined through the team
supervision model, to assist Service Coordinators with meeting the needs of the
family, as determined through assessment. Ensure child safety will occur by
serving as the safety monitor in the family through specified contact by
conducting on-going safety checks to determine if the child continues to remain
safe. Conduct assessments of the child/children and/or family's strengths and
needs related to safety, permanency, and well-being of the children. Facilitate
smooth transitional and independent living skills and services to all
youth-identified by the team.
Qualifications: High School/GED with two (2)
years experience working in the human services field. Bachelor's Degree
PREFERED. BI-LINGUAL PREFERRED. Must have VALID DRIVER'S LICENSE with no more
than 5 points against your license. |
