GENERAL SUMMARY: Responsible for and assists with assessment, planning, implementation and evaluation of patient care in a professional, confidential, and caring manner. Assists with patient care and procedures. Ensures that a follow up plan of care and educational needs are met and coordinates the flow of patients, phone calls, lab work, and nurse visits to facilitate quality patient care and customer service goals. # PRINCIPAL JOB FUNCTIONS: Commits to the mission, vision, beliefs and consistently demonstrates our core values #Performs service excellence must-haves to achieve an excellent patient/customer experience. Makes accurate and timely observations of patient#s condition and documents those changes in the patient chart accurately and according to policy. Performs a pre-visit chart review for completeness including areas of health maintenance exams, medications, tests, and visits; assists in the coordination of these tasks per provider protocols. Performs a pre-visit chart review for completeness including areas of chronic disease management. Performs a huddle with provider regarding identified questions, individualized patient care and wellness needs. Performs basic clinic nursing skills. Administers medication and refills according to policy and provider#s orders. Works efficiently and closely with other departments for the care and treatment of the patient. Assists in identifying teaching, learning, and discharge needs and communicates with the provider, patient, family, Utilization Review (UR) and Social Services Designee. Assists the provider with examinations and procedures. Explains tests and other procedures as needed for adequate patient education. Assists in the Coumadin clinic per provider protocol. #Manages all equipment correctly and safely. Identifies opportunities for performance improvement and provides input for the development of solutions #Performs other related projects and duties as assigned. Organizes, prioritizes, and assists in the coordination of patient care in stressful situations. #Maintains a neat, organized and safe environment for the patients, visitors, and MMC team. Demonstrates competency in tasks and skills required for this position. Reviews patient charts for completeness including areas of chronic disease management pertaining to visits, labs, and consultant notes; and assists in the coordination of these tasks per provider protocol. Assists in the completion of the medication refill process per physician/provider protocol. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. #Participates in meetings, committees and department projects as assigned. #Performs other related projects and duties as assigned. # EDUCATION AND EXPERIENCE: Graduation from an accredited professional nursing program required. Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. # OTHER CREDENTIALS / CERTIFICATIONS: Basic Life Support (CPR) certification required.
