This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore an individual's ability to live successfully in the community. The team approach involves structured, face-to-face therapeutic interventions that assist in reestablishing the individual's community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal.Job Duties include:Assists with the development of relapse prevention and disease management strategiesEnsures linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluationsProvides intensive case managementCompletes functional needs assessment(s) to determine the scope and anticipated outcomes to the servicesAssist with the individual's housing search including engaging landlords to rent to individuals and writing reasonable accommodation lettersAssist with connecting individuals to financial and in-kind resources to set up and maintain their householdEducation and Experience Required:A bachelor's degree in Human Services, Social Work, Special Education, or other closely related Human Services areaTwo (2) years of professional experience with the adult mental health populationResume must include degree earned (major & minor), graduation month & year, relevant employment dates including month and year, and populations served.A Caring Alternative, LLC is a CARF accredited service provider with facilities in Asheville, Hickory, Marion, and Morganton, NC. A Caring Alternative, LLC is CABHA certified and a Trauma Informed Agency.
