Self-motivated and able to work independently with minimal supervisionGood verbal and written communication skills with use of tact, diplomacy, and discretionAbility to read and comprehend written documentation and instruction related to services and medication assistance. Assistance with management of incontinence including toiletingf. Apartment organizing, laundry, and light housekeepingDocuments all care and services provided per policy and procedureAt the beginning of each shift worked, completes shift transition duties including:a. Reading electronic documentationmessages since last shift worked.b. Signing for transfer of keys, pagers, and cell phones.c. Completing narcotics counts with outgoing staff member and documenting pletes dining room responsibilities including setting tables, serving meals, and cleaning up after meals as assignedAssists residents in meeting nutritional needs by:Monitoring resident intake and completing meal tracking documentation as assignedWeighing residents and recording weight as scheduled; reporting any significant weight gainlossProviding residents with reminders to attend meals as needed, as well as setting a meal set aside when resident is unable to attend a meal in the dining roomDelivering meal trays to residents' rooms as necessaryProviding cueing or other special assistance (cutting up food, etc.) to residents who require this in order to eat adequatelyUnder the supervision of the nurse, provides assistance with medication administration including:Giving resident medication accurately as ordered, per policy and procedureDocumenting all medication assistance providedCompleting vitals (pulse, blood pressure, accuchecks, temperature, spo2) as they relate to providing medication assistance per physician's orders and nurse's instructionsImmediately reporting any medication errors made or discoveredNoting, documenting, and reporting any medication side effects or adverse reactions observed or reported by residents, as well as any resident refusals to take medicationCompletes laundry and cleaning duties as scheduled and within the guidelines establishedCarries a pager and cell phone to respond to resident calls within a timely manner and per Assisted Living policies and proceduresAssists in the provision of leisure activities, including but not limited to:Accompanying residents on outingsConducting scheduled and unscheduled leisure activitiesGathering residents for and encouraging participation in activitiesProviding residents with one-on-one attention as time allowsObserves, reports, and documents changes in resident conditionstatus through:a. Verbal report to nursesupervisormanagement.b. Completion of electronic documentation.c. Completion of incident reportstatement.Practices effective infection control measures per policy and procedure, including but not limited to:Appropriate linenlaundry handlingFrequent hand washingUtilization of Standard PrecautionsUtilization of appropriate isolation precautions per Centers for Disease Control and Prevention (CDC) guidelines and nurse instructionsMaintaining compliance with campus tuberculosis (TB) testing policyAppropriate utilization of common refrigeratorsMaintains an orderly, sanitary, and safe environment by:Reporting safety hazards immediately to management, environmental services, or a member of the Campus Safety CommitteeTidyingstraightening Wellness Center, medication carts, copy rooms, laundry rooms, and staff desk areas on a regular basisProviding assistance to residents in performing simple housekeeping tasks such as straightening apartment, removing soiled clothingpersonal care products, emptying trash, removing spoiled food, and bed making. Reporting on-going issuesconcerns to SupervisorsReporting all maintenancehousekeeping needs in a timely mannerAssists in the provision of health-related services as directed by the nurse, including but not limited to:a. Providing assistance with routine catheter care.b. Applying ointments and creams per physician's orders.c. Applying elastic stockings, ace wraps, bandages, etc. per physician's ordersd. Providing assistance with oxygen and refilling oxygen tanks.e. Collecting urine and stool specimens.Utilizes appropriate body mechanics in the completion of all job duties. Carries and utilizes a gait belt per policy and procedureAccepts responsibility for initial and on-going resident orientation and adjustment to the facilityEstablishes and maintains opensupportivecollaborative communication with peers, members of other departments and of the multidisciplinary team, residents, and families. Promotes a working environment that encourages teamwork and discourages gossipMaintains confidentiality of resident informationAssists in maintaining a safe and secure environment by:a. Receiving, screening, and directing visitors.b. Reporting any unusual situations to management andor pletes rounds, checking on the safety and well-being of residentsCompletes all mandatory training and ongoing in-services (fifteen [15] hours annually). Complies with all required training and participates in all mandatory meetings.Employee Responsibilities In EmergencyReports to the emergency panel to check the source of the alarm.
