Our unique service supports facilities to ensure enhanced quality of life for our patients.Job Description:The Behavioral Health Care Manager will be responsible for reviewing patient charts and communicating with the Clinical department and providers around any irregularities as part of chronic care management.Responsibilities:Provides assessment and care management services, including:The administration of validated rating scalesInitiates behavioral health care planning concerning behavioral or psychiatric health problemsRevisesmodifies care plans for patients not progressing or whose status changesBrief psychosocial interventions if neededOngoing collaboration with the billing practitionerMaintenance of the registrytracking sheet CConsultation with the psychiatric consultantHas a continuous relationship with the patientCollaborative, integrated relationship with the rest of the care teamConducts an interdisciplinary care plan meeting to go over the patient's beneficiary to the programQualifications:Bachelors degree in social workExperience in long-term care preferredExperience in behavioral health preferredBenefits:HealthDentalVisionCompany Sponsored Life InsuranceShort and Long-term DisabilityPaid Time OffSalary: $60,000 - $75,000EEOAAP Statement: MedElite is an equal-opportunity employer.
