Growing and established hospital is looking for of several clinical Pharmacist's to join our team of Pharmacist'e Grow With Us!Type: Full-TimeShift: 8;00 AM-4PMHours: Days (weekends when applicable)Pay: $71.79-$74.57The clinical staff pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, the clinical staff pharmacist will provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. The clinical staff pharmacist will be proficient in providing direct patient-centered care and integrated pharmacy operational services in decentralized practice settings such as the emergency department, critical care units, and general medicine or surgical floors working alongside physicians, nurses, and other hospital personnel. In addition, the clinical staff pharmacist will provide medication education to hospital staff and serve as preceptors for doctor of pharmacy students and pharmacy residents. Daily participation in quality improvement initiatives such as transitions of care, population health, and antibiotic stewardship is expected.Job SummaryLeads assigned patient service lines, clinical areas, and therapeutic programsFacilitates pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards Designs and implements stewardship activities and surveillance programsTracks and evaluates assigned pharmacy programs for operational, quality, and financial efficiency and routinely benchmarks against local and national best practicesActively identifies practice related issues which require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practiceDevelops and oversees policies and procedures for drug usage and dispensingAssures pharmacy is an integral part of the health-care delivery system and facilitates enhancement and expansion of pharmacy servicesDelivers direct patient care and clinical practiceWorks as an active member of multidisciplinary team through rounding and collaborates with healthcare providers within decentralized patient care areasIdentifies high-risk patients and implements interventions to improve quality and safetyMakes appropriate evidence-based, patient-centered medication recommendationsParticipates in the management of medical emergenciesProvides discharge medication review, reconciliation, and counseling as appropriateProvides pharmaceutical services throughout hospitalProvides accurate, safe, timely, and appropriate medication therapy based on patientage and needsCompletes critical patient monitoring and reviews patient profilechart to identify, mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failuresCommunicates effectively and appropriately with healthcare providers and caregivers and assures continuity of pharmaceutical care between shifts and among staffActively participates in stewardship activities and restricted medication programsParticipates in pharmacy operations and medication dispensingMaintains proficiency in and actively engages in operational programs and central dispensing pharmacies as appropriate for job assignmentFacilitates specialty medication dispensing procedures including but not limited tochemotherapy, parenteral nutrition, controlled substances, etc. as appropriate for assignmentFacilitates experiential education and practice advancementMaintains proficiency in preceptor roles and actively emulates educationmentorship skillsDevelops student, resident, and staff training experiences and competencies and creates new relationships for teaching and training opportunitiesPrepares clinical pearls for department meetingsPrecept doctor of pharmacy students on hospital and clinical rotationsPreceptor pharmacy resident on clinical rotationsIdentifies, designs, and implements improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costsQualificationsRequirements:Graduate from an approved School of Pharmacy.Must possess current license to practice as registered pharmacist in N.Y.SMust fulfill NY State required continuing education for pletion of 1 year Pharmacy internship.Ability to bend, stretch, and reach necessary for handlingfingering labels, vials, and apparatus used for compounding medications.Verbal ability is needed to interpret and and understand written prescriptions, formularies, and reference texts, as well as for presenting data and information on pharmaceuticals to physicians, staff and public.Graduate of an ACPE accredited College of Pharmacy with Doctor of Pharmacy degreeCompletion of ASHP accredited PGY-1 Pharmacy Residency
