What we want The Heart failure nurse practitioner (HFNP) in the Department of Primary Care & Rural Medicine, will care for patients in both independent and directed fashions under the supervision of an advanced heart failuretransplant cardiologist (AHFTC) . The patient care includes both inpatient and outpatient practices. It will be a collaborative care effort between the HFNP and AHFTC. The HFNP will share weekends and holiday calls with AHFTC in equitable manner. What you need to know Salary: Compensation will be commensurate to selected hire's experience. Schedule: This is a full-time position. Other Requirements or Factors: Some travel may be required on an as-needed basis. Needs to be comfortable with critical care and end of life patient care situations. Physical Requirements: Ability to walkmove about the distances necessary to see inpatient consults throughout the St. Joseph Hospital in Bryan TX. This is approximately 2 milesday of walking inside the hospital. Ability to liftmove medical equipment, as needed, of light to medium weights. Ability to perform as part of advanced life support team, including performing chest compressions. Required Education and Experience: * Master's degree in Nursing or equivalent combination of education and experience * One year of related experience in outpatientinpatient care Required Licenses and Certifications: * Certified Nurse Practitioner (NP) * Licensed Registered Nurse (RN) * CPR certified Required Knowledge, Skills, and Abilities: * Ability to multitask and work cooperatively with others * Strong verbal and written communication skills * Strong analytical, problem-solving, and decision-making skills * Ability to work with sensitive information and maintain confidentiality * Ability to use and manage an Electronic Medical Records (EMR) system Preferred Qualifications: * Current BLS and ACLS certifications * Completed training in a HFNP program * More than 1 year of experience in outpatient HF care and more than 1 year of experience in inpatient HF care * Experience with heart transplant patient care * Experience with left ventricular assist device patient care Responsibilities: * Inpatient Care - Provides direct patient care, assessments, counseling, preparation, and treatment. Performs all necessary tasks to care for inpatient heart failure patients. This includes but is not limited to: performing history & physical, researching all relevant imaging, researching all relevant test data, researching all relevant family-social economic information, making diagnoses, developing treatment plan, completing all necessary follow-up visits, and presenting all patient care information to AHFTC. Prescribes medicines and orders diagnostic tests, as needed, upon diagnoses of illness or injury, under the direction of a physician and within the limitations of Nurse Practitioner regulations. * Outpatient Care - Performs all necessary tasks to care for outpatient heart failure patients. This includes but is not limited to: performing history & physical, researching all relevant imaging, researching all relevant test data, researching all relevant family-social-economic information, making diagnoses, developing treatment plan, completing all necessary follow-up visits, and presenting all patient care information to AHFTC. Documents patient care. Maintains and monitors patient records, assesses health care needs, and provides patient education. Interprets lab andor x-ray results Makes necessary follow-up calls, pre-authorization requests, peer-to-peer appeals, helps with patient assistance application, and coordinates with external medical entities. Performs follow-up care in conjunction with other support staff. Prescribes medicines and orders diagnostic tests, as needed, upon diagnoses of illness or injury, under the direction of a physician and within the limitations of Nurse Practitioner regulations. * Flexible Support - Supplements outpatient or inpatient care when needed. Performs tasks such as helping in development of plans, administrative functions, compliance functions, care improvement functions, teaching functions, and research functions as directed by the AFHTC.
