At Houston Methodist, the Advanced Practice Provider (APP) Post Graduate Fellowship position is a part of a twelve-month program with rotations in assigned specialty areas. This position will be given the opportunity to learn and improve assessment and diagnostic skills, as well as treatment implementation through medication management, procedures, and other therapies in the context of specialty care. The APP Provider Fellow position will be responsible for providing direct patient care in a variety of team-based settings with the goal to transition to independent practice upon completion of their fellowship. PEOPLE ESSENTIAL FUNCTIONS Provides patient care in assigned department by assessing and diagnosing clinical conditions, developing treatment plans and documenting appropriately. Uses therapeutic communication to establish a relationship with patients, families and interprofessional health care team to collaborate on the plan of care. Communicates in an active, positive and effective manner to all interprofessional team members and reports pertinent patient care and family data in a comprehensive and unbiased manner, listens and responds to the ideas of others. SERVICE ESSENTIAL FUNCTIONS Provide direct patient care services, including performing physical assessments, diagnosing and managing acute and chronic conditions, prescribing medications, ordering and interpreting diagnostic tests, and developing and implementing treatment plans in their respective specialty area(s). Advocates on patient/family's behalf to identify and resolve clinical and ethical concerns, utilizing appropriate resources. Obtains superior patient satisfaction scores via standard patient satisfaction survey tool. QUALITY/SAFETY ESSENTIAL FUNCTIONS Performs essential functions of assessment, diagnosing, treatment, and management of patients in assigned setting, including accurate and timely documentation, intake assessments, treatment planning and discharge planning, as appropriate. Collaborates with physicians, nurses, and other members of the healthcare team to ensure comprehensive and coordinated patient care. Actively participates in interprofessional team meetings, case discussions, and care planning. Engages in team based interdisciplinary care. FINANCE ESSENTIAL FUNCTIONS Self-motivated to independently manage time effectively and prioritize daily tasks. Maintains awareness of their direct and indirect financial impact on the organization. GROWTH/INNOVATION ESSENTIAL FUNCTIONS Keeps up-to-date with current evidence-based practice guidelines and professional standards. Seeks guidance and feedback from preceptors and healthcare professionals to enhance clinical skills and knowledge. Attends educational sessions, labs, and lectures to further develop skills in their respective specialty. Identifies own learning needs and seeks continuing education opportunities to meet those needs. Consults with healthcare team and mentors for guidance in self-identified areas of need. EDUCATION Master of Science in Nursing WORK EXPERIENCE Two years of previous RN practice LICENSES AND CERTIFICATIONS - REQUIRED RN - Registered Nurse - Texas State Licensure and/or Compact State Licensure within 60 days OR RN-Temp - Registered Nurse - Temporary State Licensure within 60 days AND APRN - Advanced Practice Registered Nurse - Texas Board of Nursing AND Magnet - ANCC Recognized Certification -- Certified by American Nurses Credentialing Center (ANCC) or American Association of Nurse Practitioners (AANP) as Nurse Practitioner based on applicable patient population, i.e., FNP, AGNP, NNP, PMHNP, WHNP OR AANP - Credentialed member, American Academy Of Nurse Practitioners (AANP) -- Certified by American Nurses Credentialing Center (ANCC) or American Association of Nurse Practitioners (AANP) as Nurse Practitioner based on applicable patient population, i.e., FNP, AGNP, NNP, PMHNP, WHNP AND BLS - Basic Life Support (AHA) AND ACLS - Advanced Cardiac Life Support (AHA) -- (not required for Employee Health, Employee Health Well-Being, Primary Care and Telemedicine departments) AND AP - Prescriptive Authority - Texas Board of Nursing AND DEA - Narcotics License (DEA) within 6 months KNOWLEDGE, SKILLS, AND ABILITIES Demonstrates the skills and competencies necessary to safely perform the assigned job, determined through on-going skills, competency assessments, and performance evaluations Sufficient proficiency in speaking, reading, and writing the English language necessary to perform the essential functions of this job, especially with regard to activities impacting patient or employee safety or security Ability to effectively communicate with patients, physicians, family members and co-workers in a manner consistent with a customer service focus and application of positive language principles Demonstrates advanced assessment, diagnosis, treatment planning & interventions in the field of psychiatry SUPPLEMENTAL REQUIREMENTS WORK ATTIRE Uniform No Scrubs No Business professional Yes Other (department approved) Yes ON-CALL* *Note that employees may be required to be on-call during emergencies (ie.
