A nonprofit community services organization in New York City is currently seeking a new Case Planner for a promising opportunity with their Foster Care program.About the **Start ** ASAPAssignment ***** Ongoing (Contract to Hire potential)***** Monday to Friday***** 9am to 5pm** Hybrid (2 days in the field & 3 days onsite)Patient ** Foster Care (Ages 0-21)** Electronic State System ConnectionsThe Case Planner will be responsible **Work with Foster Parents and Biological Parents to plan childs permanencyProvide support and case management services to foster and adoptive children in foster homesPrepare and send referral packages for necessary services to appropriate community resourcesEnsure that service plans are implemented for all children careSchedule and supervise visitations between children and familiesParticipate in all Family Team Conferences (FTC) Attend all court hearings, hospital visits, etc. as neededParticipate in and ensure follow up from FTC meetingsPerform other duties, as needed*2+ years of relevant work experienceBachelors Degree in Social WorkKnowledgeable about working with foster care youth and children and families at riskMicrosoft Office proficientWorking knowledge of OCFS ConnectionsSolid assessment and documentation skillsGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired **Bilingual (English and Spanish)
