A community services organization in New York City is actively seeking a new Case Manager to join their growing Women's Shelter in Brooklyn.About the Opportunity:Start Date: ASAPAssignment Length: OngoingSetting: Women's ShelterSchedule: Monday to FridayHours: 9am to 5pmResponsibilities:The Case Manager will:Conduct an initial assessment of the family within 48 hours of arrival to the residenceComplete the Family Case Assessment Form, Initial Service Plan, and all initial intake documentationInitiate referrals for necessary emergency servicesInitiate/complete intake information (CARES data entry and supporting documentation) in the Uniform Client ChartEnsure CARES data and supporting documents are printed and filed in designated sections in the client's chartConduct orientation with clients and review the facility rules, clients' rights and responsibilities, grievance/complaint process, emergency disaster procedures, etc. Assist clients with transition to assigned residential unit and housekeeping items.Provide comprehensive case management, including crisis intervention, counseling services and other appropriate support servicesMaintain client data in the Uniform Client ChartAssist families in obtaining income and public benefits (i.e., Public Assistance; SSI, etc.)Establish and refer clients to appropriate medical and mental health services, education/job training programs, legal and advocacy services and other community resources as neededPerform other related duties, as assigned Qualifications:1+ year of Social Services experienceBachelor's DegreeCase Management experienceExperience with Substance Abuse Social Work and treating clients with mental health issuesDesired Skills:2+ years of experience with Homeless Adults and Substance Abuse issuesBachelor's Degree in Social WorkCASAC certification or CASAC-TBilingual (English and Spanish)Clinical backgroundFamiliarity with EBP such as strength-based Case Management, Motivational Interviewing, Trauma-informed Care, Critical Time Intervention, Family-centered Case Management, Boundary setting, and Cultural-reflexivityFamiliarity with entitlement systems and procedures Working knowledge of CARESExperience with standardized assessment tools
