here:Under the direction of the Occupational Therapist the COTA will provide habilitation or rehabilitation occupational therapy services to clients in their residence.Performs all occupational therapy procedures planned and supervised by a qualified Occupational Therapist in accordance with the plan of careAssists and instructs clients in pushing wheelchair, supporting or lifting, positioning in and out of bed and other activities of daily livingAssists in specialized motor or self-help activities, constructs supportive or assistive devicesObserves client for signs of distress and adjusts activities accordinglyParticipates in case conferences, staff meetings and in-service programs as requiredPrepares clinical notes and progress reports and submits them for review by the occupational therapist to be incorporated into the client's clinical recordPractices with respect for individual, cultural, and spiritual differencesConsistently demonstrates competency with occupational therapy skills according to personal and legal scope of practiceEvaluates the home environment and make appropriate recommendationsInstructs client, family and other team members in proper use of assistive equipment, prosthetic devices and adaptation devicesEvaluates effectiveness of teaching and modifies accordinglyPromotes client/family autonomyRelays significant changes in status to physician and other members of the team in a time period consistent with client needDocuments client charts, records, clinical notes, etc., legibly and according to documentation guidelines and standardsReports any changes in client's condition to the Occupational Therapist or RNCompletes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Therapy AssociationOne (1) year experience as a COTA in any acute or post-acute settingCurrent valid license as an Occupational Therapy Assistant (in good standing) in the state(s) of practice Valid
